​In order to participate in Phase 1 of the LBNS Return to Play Plan, bowlers need to sign the appropriate waiver forms and declaration of compliance before they can participate in the activity.
There are three forms available of which participants must have signed the appropriate forms in advance of participating. Click on the heading on the right side of the page to be directed to the appropriate form.

• Declaration of Compliance (For All Participants)
• Release of Liability, Waiver of Claims and Indemnity Agreement (For Participants Over Age 18)
• Informed Consent and Assumption of Risk Agreement (For Participants Under Age 18 signed by the parent or guardian.
